At Stony Brook Medicine, a Clinical Systems Application Specialist is a valuable member of our team who will develop and support all applications/systems that the Clinical Information Systems department is responsible for. The incumbent may be assigned to a project team as part of technical build resource, or support any production systems to ensure the availability of the applications. The Clinical Systems Application Specialist may also oversee the work of other analysts as needed.

The Clinical Systems Application Specialist will be responsible for build in Cerner Millennium, and also investigating and resolving Cerner application issues using core Cerner tools such as Bedrock, Discern, Core Code Builder, Core Event Manager, and Ops View Scheduler. Daily responsibilities will include researching and building new functionality as needed, working with clinical end users and the vendor to determine the best and most innovative solutions.

Duties may include the following but are not limited to:

• Provides support to Clinical Information systems applications which includes our organizations EHR and cross applications used in the clinical setting
• Provides technical and strategic input to leadership, reviews, analyzes and implements change/enhancement requests to help drive transformational changes.
• Plays an active role as a technical expert during application development and participates in brainstorming and design discussions with team members which includes participation in new projects when appropriate.
• Provides technical guidance and performs investigation to help resolve application issues including cross application challenges in order to maintain stability and reliability of all systems.
• Works closely with Project Managers on all aspects of project development and keeps management informed of project status.
• Provides assistance in testing system functionality as well as validating application integrity to ensure that current and new applications are technically sound
• Participates in Cerner application upgrades and implementations.
• Debug system problems as needed (his/her own, as well as programs written by others).
• Participates in all global and departmental process and system design activities
• Adheres to coding guidelines and standards of Information technology and documents all work according to standards
• Adheres to customer service standards.

Our nephrology practice is seeking a board-certified nephrologist to join our team full time. The successful applicant will work with one other nephrologist and two nurse practitioners. Compensation is above average and depends in part on experience.

JOB RESPONSIBILITES:

• Provides health care to patients including diagnosis of nephrology conditions such as kidney disease, kidney failure, diabetic disorders, hypertension.
• Obtains medical histories, performs physical examinations, and makes diagnosis, and orders appropriate, standard laboratory and diagnostic procedures as necessary.
• Evaluates procedure results. Provides preventative as well as acute care.
• Provides continuity of care by developing and implementing patients plan of care.
• Provides patient education about underlying medical condition, self-management of acute or chronic illnesses and tailors instructions to patients individual circumstances.
• Instructs and counsels patients regarding therapeutic regimens, promoting wellness and health maintenance.
• Prescribes medications.
• Documents exam and progress notes in the Electronic Medical Record. Provides referral letters as needed.

The Nurse Practitioner/Physician Assistant to work collaboratively with cardiologists in an inpatient and outpatient setting.

Hours:Full time, must be able to rotate at the hospital, rotate weekends & must have availability to be on call.

Under the direction and supervision of a licensed physician, provides cardiac care to patients.

Perform comprehensive inpatient/outpatient visits which include:

Outpatient:

• Conduct comprehensive or episodic health history and physical assessment of patients with acute, chronic and/or cardiac patients with potential health problems.
• Establish medical diagnoses. Write specific orders within the limits of the practice agreement/scope of practice.
• Perform stress testing, provide appropriate direction to office staff regarding patient care issues (triage calls etc. Manages or independently performs evaluative and treatment necessary to provide an appropriate response to life threatening emergency situations until the arrival of emergency response teams.
• Communicates effectively with referring and primary health care providers. Manage independent outpatient clinical practice and coverage for collaborating physicians, including rotating on call weekday/weekend night and weekend coverage.

Inpatient:

Our Pulmonology practice is seeking a board-certified or board-eligible pulmonary, critical care physician with sleep medicine board-eligibility or certification preferred but not required to join our growing team. The successful applicant will work with nine pulmonologists offering state-of-the-art pulmonary, critical care and sleep medicine services. We are incredibly flexible and welcome recently trained fellows. Compensation is above average and can expand further in part to experience.

Position Summary:

Accountable for engaging with patients and their family in telephonic-based Care Management. This role will provide care management to different targeted populations in various program arrangements, including but not limited to, management of care transitions, longitudinal care management and disease based care management. Coordinates with Skilled Nursing Facilities and other post acute providers. Works under direct supervision and is a part of an interdisciplinary Care Management team. Responsible for managing the coordination of behavioral health services and addressing the social determinants of the targeted population. Responsible for end-to-end comprehensive care management, overall care planning and performance metrics for assigned caseload.

General Functions:

• Engages with patients and their family, and members of the health care delivery team about goals of care and the care plan; knowledgeable about level of care needs, community resources, and insurance benefits, so that appropriate referrals and resource linkages can be made to support ongoing self-care needs.

• Liases with members of the Stony Brook Post Acute Care Collaboration (PACC) around progression of care for assigned patients
