JOB SUMMARY: Assist the Quality Performance Measurement Unit staff with day-to-day operations to ensure optimum operations.

JOB DUTIES:

• Review patient charts for missing data/gaps in care
• Check lab portals, Zwanger and Cologuard to assess measure compliance
• Call patients to gather information and/or recommend follow-up appointments
• Assist with running reports and pulling data for ACO measures through Cerner and HealtheRegistries
• Create chase lists for varying quality measures
• Communicate through the EMR with practice administrators and other members of the team
• Answer calls to the SBCM concierge line
