Responsibilities:

• PCMH tracking- referrals, lab, radiology orders, patient result outreach
• Attend PCMH meetings, reporting
• New patient orientation, records tracking, collection and preparation
• NYSIIS vaccine reporting- and lead reporting
• Well visit recall patient outreach
• Urgent care visit outreach and appointment follow up
• Hospital discharge tracking and follow up appointments
• Receptionist duties- including heavy phones, appointments, copay collection
• Chart preparation-printing and paperwork prep for up-coming appointments
• Specialist concierge contact as needed
• Record scanning
• Sending out record releases to practices and receipt tracking
• Sending and confirming all Verisma record releases
• Creation of behavioral health patient care plans as directed
• General front desk office duties as needed
• OT/PT scripts, referral creation and filling out school forms or other paperwork
