Job Description:

A Nurse Practitioner to work collaboratively with Cardiologists in an office setting.

Job Functions:

• Under the direction and supervision of a licensed physician, provides cardiac care to patients.

• Perform comprehensive outpatient office visits which include: obtaining medical history, performing physical examination, establishing diagnosis, and developing the patient’s plan of care, with a focus in congestive heart failure. The nurse practitioner will be required to complete all documentation, as well as order medications, diagnostic testing, and review the testing results. He/she will provide patient education about the underlying medical condition, and instruct and counsel patients regarding therapeutic regimens, promoting wellness and health maintenance.

• In addition to the office visits, the Nurse Practitioner will perform other duties as assigned by supervising physicians, such as completing pre-catheterization education and forms, preauthorization and disability forms.

• The Nurse Practitioner will administer diagnostic cardiac testing, including stress testing

• The Nurse practitioner will assist in patient phone calls.
