Summary: The EMR Abstractor will collect and process key information from a patient's medical record and transcribe that into discrete fields in the EMR. Abstraction includes medications, allergies, past medical problems, procedures, surgeries, family history, social history, and pregnancies based on the provider's office notes.

Job Duties & Essential Functions:

• Receives selected identified paper medical records to be abstracted daily.
• Collects key information from the patient's medical record and transcribes that into discrete fields in EMR.
• Abstraction includes medications, allergies, past medical problems, procedures, surgeries, family history, social history, clinical alerts, and pregnancies based on the provider office notes.
• Familiar with all parts of medical record and chart review.
• Reviews abstract informtion timely.
• Abstract information with a high degree of accuracy.
